John Imhoff My dad has been here for about 2-weeks. The care he gets is very good. He is comfortable, clean, interacted with and challenged to try and get better. Very happy with his care.

Katie Cook I just drove all the way up here from Georgia to see my father for what I believe to be the last time. He has been a resident of Steuben Center since March 1st this year. His health has declined steadily since hes been there. He weighed 165 lbs when he arrived and now he is down to 123 lbs. Seeing with my two eyes that he is now a skin covered skeleton obviously neglected by the facility for months. Our family has been reaching out and trying to advocate for him because he has dementia and cannot advocate for himself. No one is ever available to speak with us and calls back are extremely rare. When you finally do get to speak with someone and ask your questions they do not have answers but they tell you they will find the answers and get back to you. Rarely do you ever hear back. They take full advantage when someone cannot stand up for themselves and what is right. Walking down the halls hearing people crying out for help with not a single aid in sight. I was there for 2 hours. I saw two workers there when I first arrived but when I went back out of his room 10 minutes later they were gone. I walked around and around the entire unit and could not find anyone except for the people that were in the dietary area. I know they saw me but they looked away and acted real busy. Speaking of which Im pretty certain dinner is supposed to be served at 5:00. When I showed up, my dad was begging for food. I ran down the road and got him some and when I brought it back he was ravenous and ate it all as fast as he could. These people at the facility keep telling me that he is not eating that he has failure to thrive and now a new cancer diagnosis so his death is imminent. My dad has had a history of depression and this place is the most depressing setting I have ever seen for someone in his condition. There wasnt even a cup in his room for water. The ombudsman program is a freaking joke. They cannot help you FYI. My family has been begging everyone we can speak to in the office to transfer him out of there ASAP. I WILL NOT let my father die in this facility. If any of you incompetent employees are reading this just know that I have contacted the health department, the attorney general and the senators office. You will definitely all be aware of the next step that I take. People should band together and start a class action lawsuit against this place. If they keep procrastinating on transferring my father to a facility closer to home we will have no choice but to take him out of there AMA and take him to a hospital where they can find somewhere else for him to go. They cannot provide him comfort care. They cannot provide any kind of comfort to the residents there. The residents get treated worse than animals at an animal shelter. Reading all of these negative reviews. They quite obviously have been getting away with this type of treatment for years and dont give a single f about your concerns or your loved ones well-being.

India Samara I reported this place to the state twice and tried to take action with lawyers. They repeatedly overcharged (against the law) for medical records and would not comply with lawyer requests for records. They erased my mother’s record of them breaking both her arms without licenses. Admin will not call back and they have a new person running the facility every year or so it seems.

Arlene Sanders This place should be shut down.
We pulled our family member from here - went for rehab and was so mismanaged and neglected, he died a week after leaving. He died with a necrotic hole in his coccyx that he got while there, his surgical incision on his head was infected, the other residents said staff would take 3 minutes to feed him if we weren’t there, he seldom received therapy, he was so dehydrated frequently, urosepsis, no one would listen to our concerns.. we offered to arrange transport home, they insisted on arranging it… he was sent home in a gerichair, unresponsive with vomit all over him. They were going to arrange services and equipment - NO WHERLCHAIR, NO SERVICES!! He went for rehab, not end of life care. Will be reporting to the state.
I could go on and on with all the horrible care he experienced but I’ll just report it to the state and leave it at, if you love your family member, I would not send them there.
The only thing pulling this rating number up, is the staff giving 5 star reviews

Josh Sprague Good to see the state is taking action against these nursing homes that inflict neglect, harm, and unreasonable care for their patients. It’s horrible to hear that your organization funneled Medicare/medicaid funds to the top of the top and lined executives pockets. You people should be ashamed of yourselves. As a healthcare worker I pity any neglect or harm to patients. Your organization is disgusting! Good riddance! Shameful!
